Argatroban infusion rate
Starting dose and pump rate for argatroban in HIT, with hepatic and critical-illness reductions.
Inputs

Patient values are shared with every calculator.
When to use
- Suspected or confirmed heparin-induced thrombocytopenia.
Pearls and pitfalls
- Label start: 2 mcg/kg/min; 0.5 mcg/kg/min with moderate hepatic impairment.
- Many centers start 0.5 to 1.2 mcg/kg/min in heart failure, multiorgan failure, or after cardiac surgery.
- Target aPTT 1.5 to 3 times baseline (not above 100 s); maximum 10 mcg/kg/min.
- Argatroban raises the INR; follow the label when overlapping with warfarin.
Why use
Converts the weight-based dose to a pump rate with the reduction built in.
Rate (mL/h) = dose (mcg/kg/min) × weight × 60 ÷ 1000 ÷ concentration (mg/mL)
