When to use
  • Loading theophylline or aminophylline (rarely needed today).
Pearls and pitfalls
  • Target 5–15 mg/L for most adults (label); toxicity rises above 20.
  • Smoking, rifampin, and phenytoin increase clearance; heart failure, liver disease, ciprofloxacin, and erythromycin decrease it.
Why use

Handles the salt factor and the level already on board.

Load (theophylline) = 0.5 L/kg × IBW × (target − current); aminophylline = theophylline ÷ 0.8
