Parenteral nutrition macronutrients
Daily amino acids, dextrose, and lipid from calorie and protein goals.
Inputs
3 of 4Dosing weight (adjusted in obesity).
kcal/kg/day
g/kg/day
%
Patient values are shared with every calculator.
When to use
- Writing or checking an adult PN order.
Pearls and pitfalls
- Critically ill adults: 25 to 30 kcal/kg/day and 1.2 to 2 g/kg/day protein (more in burns, trauma).
- Lipid usually 20% to 30% of non-protein calories; at most 2.5 g/kg/day (1 g/kg/day for soybean oil emulsion in the ICU).
- Count propofol (1.1 kcal/mL) as lipid calories.
Why use
Shows every gram and calorie so the order can be checked line by line.
Protein kcal = g × 4; lipid kcal = g × 10 (20% emulsion 2 kcal/mL); dextrose g = remaining kcal ÷ 3.4
