Warfarin weekly dose adjustment (INR goal 2–3)
Suggested change to the total weekly warfarin dose from the current INR.
Inputs
0 of 2mg/week
When to use
- Routine outpatient INR management with a goal of 2 to 3 and no bleeding.
Pearls and pitfalls
- One slightly out-of-range INR (within 0.5 of goal) often needs no change; look for a cause first (missed doses, diet, new drugs, illness).
- Recheck in 1 to 2 weeks after a change.
- INR above 4.5 or any bleeding needs individual assessment, possibly vitamin K.
Why use
Changes the weekly total by a set percentage, the approach used by most anticoagulation clinics.
New weekly dose = current weekly dose × (1 + % change from the INR band)
