When to use
  • VTE treatment or prophylaxis with enoxaparin.
Pearls and pitfalls
  • Use actual body weight for treatment; consider anti-Xa monitoring above about 150 kg, in pregnancy, and with CrCl below 30.
  • BMI 40 or more: prophylaxis 40 mg every 12 h is commonly used (off-label).
  • Platelets at baseline and periodically (HIT).
Why use

Applies the label's renal adjustment and rounds to real syringe sizes.

Treatment 1 mg/kg every 12 h (CrCl < 30: every 24 h); prophylaxis 40 mg daily (CrCl < 30: 30 mg daily)
