When to use
- Critically ill patients where continuous infusion is preferred.
Pearls and pitfalls
- Target Css 17–25 mg/L corresponds to AUC24 400–600.
- Load with 20–35 mg/kg (guideline), then start the infusion.
- Check a random level 24 h after starting; adjust proportionally (new dose = old × target ÷ measured).
- Watch IV compatibility on the shared line.
