KDIGO acute kidney injury staging
AKI stage from creatinine rise, urine output, and dialysis.
Inputs
3 of 5Rose 0.3 mg/dL or more within 48 h
Urine output
Started dialysis
When to use
- Recognizing and staging AKI, and flagging drugs to adjust or hold.
Pearls and pitfalls
- Creatinine lags: estimate clearance with the unstable-renal-function tool while it rises.
- Review nephrotoxins (NSAIDs, aminoglycosides, vancomycin plus piperacillin-tazobactam, contrast) and renally cleared drugs.
Why use
A standard stage to document and to trigger renal dose review.
Stage by the worst of: creatinine ratio to baseline (or rise ≥ 0.3 mg/dL in 48 h), urine output, or dialysis
