Log experiential hours, de-identified patient encounters, and skills for pharmacy IPPE and APPE, nurse practitioner, and physician assistant rotations. Free, with CSV export and a printable summary for your preceptor to sign.
Never record patient identifiers. No names or initials, dates of birth, medical record or room numbers, or anything else that could identify a patient. Encounters are logged with picklists only, and store the rotation week instead of a date. There is no place to type about a patient, by design.
Your school or site may require its own system (for example E*Value, Exxat, CORE ELMS, or Typhon). Use this log as your personal record and check what your program accepts.

Required hours vary by school and accreditor (for pharmacy, ACPE Standards require at least 300 IPPE hours and 1,440 APPE hours); enter your program's numbers. This log is a personal record, not an official school or board record.
