Educational check from entered facts, not an official measure calculation. Printed from ClinDose (clindose.com/value-based-care/quality).

    How it works

    1. The measurement period is January 1 to December 31 of the "check as of" year. Results dated after the check date don't count yet. The patient is assumed to have a qualifying visit this year.
    2. Each measure applies its eCQM age range and denominator exclusions (hospice, palliative care, nursing home residence or frailty with advanced illness at 66 and older, and measure-specific ones such as ESRD for blood pressure or mastectomy for breast screening).
    3. The numerator logic follows the 2026 eCQM specifications: the most recent A1c (inverse: above 9% or missing is a gap), the most recent blood pressure below 140/90, a statin for any of the four risk groups, screening look-back windows (mammogram since October 1 two years before; colonoscopy 10 years; stool DNA with FIT 3; sigmoidoscopy and CT colonography 5; FIT this year; cytology 3 years; HPV 5 years at 30 and older), depression screening with follow-up when positive, and tobacco screening with cessation help for users.
    4. Influenza and pneumococcal status follow the CDC adult immunization schedule, because CMS retired those eCQMs (CMS147 and CMS127) after 2023. Use of high-risk medications in older adults (CMS156) is shown as a review item only: its drug list is licensed and not reproduced.
    5. Due dates are the end of the measurement year for open gaps, and the guideline interval (for example, 2 years after a mammogram) for met screenings.
    6. HEDIS measures with similar names (from NCQA) have their own licensed specifications, which can differ from the CMS eCQMs; they are not reproduced here.
